About the position
The Medical Care Counselor position at Catholic Charities of Long Island involves providing health-related advocacy and support for individuals residing in group homes for developmentally disabled adults. The role emphasizes compassionate care and effective communication with healthcare providers to ensure the well-being of residents.
Responsibilities
- Provide continuous protective oversight to individuals in group homes.
- Schedule, transport, and accompany individuals to medical appointments.
- Facilitate communication with healthcare providers regarding individuals' health changes.
- Maintain health-related documentation, including lab results and medication prescriptions.
- Communicate daily with nursing and residential management about individuals' medical status and treatment plans.
- Manage medication inventory, including prescriptions and first aid supplies.
- Provide companionship visits to hospitalized individuals as required.
Requirements
- High school diploma or GED.
- Experience working with developmentally disabled adults preferred.
- C.N.A. or equivalent training is helpful.
- Valid NYS Driver's License.
- Ability to transfer and transport individuals to/from medical appointments in all types of weather.
- Detail-oriented with strong organizational and interpersonal communication skills.
- Willingness to provide compassionate, person-centered care.
