About the position

As a Care Coordinator/Medical Case Manager at Clarvida, you will support clients of all ages with complex health needs, helping them to create and implement a Health Action Plan. This role involves monthly home visits to assist clients in achieving their health goals, coordinating care with healthcare teams, and linking clients to community resources. The position promotes skillset growth and supports clients in navigating healthcare services independently.

Responsibilities

• Create and implement Health Action Plans with clients.
• Conduct monthly home visits to monitor progress and support clients in achieving their health goals.
• Coordinate care needs with healthcare team members.
• Act as an advocate and point of contact for clients.
• Link clients to community resources and services.
• Promote skillset growth toward recovery and improved health status.

Requirements

• Bachelor's Degree in Human Services, Social Services, Nursing, or a related field.
• Two years of experience in nursing, healthcare, behavioral health, or human services (paid or volunteer).
• Valid driver's license with a clean driving record and auto insurance.
• Satisfactory results of fingerprinting and background checks.
