About the position

The Medical Case Manager at Maryville Academy is responsible for the collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for clients in the Children's Healthcare Center. This role focuses on meeting the comprehensive health needs of medically fragile children and their families, ensuring quality care and effective resource connection.

Responsibilities

  • Use a patient-centered, collaborative partnership approach as provided in the Individual Care Plan.
  • Facilitate self-determination and self-care through advocacy, shared decision making, and education.
  • Practice cultural competence with awareness and respect for cultural diversity.
  • Promote use of evidence-based care as available.
  • Promote optimal client and family safety.
  • Successfully link family and client with community resources.
  • Assist with navigating the healthcare system to achieve successful care in accordance with the care plan.
  • Provide initial assessment and Individual Care Plan to appropriate third parties.
  • Promote quality outcomes and assist in the measurement of those outcomes.
  • Support and maintain compliance with federal, state, local, and organizational rules and regulations.
  • Conduct a comprehensive assessment of the client's health and psychosocial needs on intake.
  • Coordinate care with the client, family, caregiver, primary care physician/provider, and other healthcare providers.
  • Maintain important relationships with discharge planners and referring personnel at children's hospitals.
  • Facilitate communication and coordination among the interdisciplinary team to minimize fragmentation of services.
  • Educate the client, family, and/or caregivers about treatment options and community resources.
  • Assist in the marketing of the agency and CHC to referral sources and outside agencies.
  • Empower the client/family to explore options of care and alternative plans.
  • Assist the client and family in safe transitioning of care to the next appropriate level.
  • Prepare the client and family for discharge, coordinating necessary resources.
  • Perform public relations and marketing assignments as assigned.
  • Plan and carry out family support meetings.
  • Perform other related duties as assigned.

Requirements

  • Bachelor's Degree in nursing or social work required, Masters preferred.
  • Experience with case management preferred.
  • Networking abilities demonstrated by work experience with Chicagoland children's hospital units.
  • Bilingual (Spanish speaking preferred).
  • Understanding of pediatric disease and illness, psychosocial interventions, and child welfare mechanisms.
  • Excellent verbal, written, and interpersonal communication skills.
  • Understanding of Maryville CHC Program and mission.
  • Proficiency with MS Windows and Office.
  • Experience with an electronic healthcare record system preferred.

Nice-to-haves

  • Bilingual (Spanish speaking preferred).
  • Experience with case management preferred.
