About the position

The Medical Case Manager plays a crucial role in enhancing traditional care teams by improving health knowledge and self-sufficiency among clients diagnosed with HIV/AIDS. This position focuses on providing extensive social services support, helping individuals navigate access to community services and resources, and ensuring they receive the necessary care and education to improve their health outcomes.

Responsibilities

  • Completes patient assessments and reassessments utilizing a strength-based model.
  • Provides health education, risk reduction, and adherence education to patients to assist in engagement and retention.
  • Conducts outreach to re-engage patients in care.
  • Participates in ongoing training and regional collaboratives to enhance knowledge and skills.
  • Participates in quality improvement trainings and activities to improve patient care.
  • Demonstrates cultural competency in working with diverse populations.
  • Develops, implements, and updates care plans.
  • Facilitates enrollment in benefit and entitlement programs.
  • Participates in case conferences presenting patient needs within a strength-based model.
  • Develops linkages and refers patients for additional supportive services.
  • Conducts intakes and verifies and documents program eligibility.
  • Supports linkage to care and treatment adherence by educating clients about services and medication options.
  • Facilitates the development of communication and client empowerment to interact with healthcare/social service systems.
  • Supports adherence to treatment by providing structured educational sessions on chronic diseases and HIV.
  • Delivers health information using culturally appropriate terms and concepts.
  • Provides informal counseling support and follow-up.
  • Supports retention in care and reduces barriers by collaborating with medical, behavioral health, and social services providers.
  • Documents activities in electronic health records.

Requirements

  • Bachelor's degree and/or 5 years' experience working with HIV client services.
  • HIV/AIDS/STI knowledge including prevention, infection, and treatment.
  • Basic computer literacy skills (e.g., TEAMS, Microsoft Word, email).
  • Superior written and verbal communication skills.
  • Ability to work in a culturally diverse environment.
  • Knowledge of eClinical Works (eCW) and Aids Institute Reporting System (AIRS) is a plus.
  • Bilingual - Spanish is a plus.
