About the position

The Medical Case Worker at Maryhaven plays a crucial role in supporting individuals with intellectual disabilities and mental health issues by ensuring their medical needs are met through community resources. This position involves monitoring and accessing services, transporting residents to medical appointments, and maintaining communication with medical providers and families to promote the well-being of the residents.

Responsibilities

• Monitor and access services to meet the medical needs of residents through community resources.
• Transport and accompany adults with special needs on medical visits.
• Provide and return documentation for filing to residence.
• Communicate with manager/supervisor regarding resident's medical status.
• Maintain positive communication with medical/day providers and families.
• Provide accurate information to providers regarding health needs of the residents.
• Ensure that medical documentation is accurate, complete, and follow-up is done as necessary.
• Maintain knowledge of consumer medical insurance.
• Ensure that proper billing information is provided at each medical visit.
• Maintain appointment calendars at each assigned residence, with copies to assigned RN and Medical Case Worker Supervisor.
• Schedule and follow-up on consumer medical appointments as dictated by health needs, physician recommendations, and program policies.
• Provide staffing coverage in residence for those consumers unable to attend day programming.

Requirements

• High school diploma or GED required.
• Valid NYS Driver's License required.
• 1 year of experience working with special needs population required.

Nice-to-haves

• 1 year of caregiving experience preferred.
• 1 year of experience working with people with developmental disabilities preferred.
