About the position

The Medical Director at Accompany Health plays a crucial leadership role in overseeing Integrated Care Teams that provide holistic, patient-centered care to low-income patients with complex medical, behavioral, and social needs. This position involves direct clinical supervision, management of advanced practice clinicians, and collaboration with a multidisciplinary team to ensure high-quality care delivery both in-home and virtually. The Medical Director will also engage in strategic implementation of clinical innovations and quality improvement activities to enhance patient outcomes.

Responsibilities

  • Lead teams of advanced practice clinicians (APCs), Physicians, Community Health Workers (CHWs), Patient Experience Navigators (PENs), RNs, behavioral health care managers (BHCs), social workers, pharmacists, and pharmacy technicians in an integrated care model.
  • Directly manage and oversee advanced practice clinician team managers and physicians within your market, providing clinical mentorship and performance management.
  • Partner with operational and nursing leadership to ensure appropriate clinical coverage for inbound triage and urgent needs.
  • Drive strategic implementation and integration of clinical innovations within your market to improve patient quality of care and efficiency.
  • Monitor, evaluate, and implement quality improvement activities, including review of patient and care team quality of care and safety concerns.
  • Lead the hiring, onboarding, and continuing training of clinical team members within your market.
  • Participate in the development and delivery of educational content for clinical care team members.
  • Assist in developing relationships and collaborative workflows with network providers and community partners.

Requirements

  • M.D., D.O., N.P., or P.A. graduate
  • Board certification in Internal Medicine, Family Medicine, or Medicine-Pediatrics required for M.D. or D.O.
  • Experience building and leading multi-disciplinary teams and supervising clinical team members.
  • Experience with change management and process improvement theory and practical application.
  • Comfort with working in an electronic medical record.

Nice-to-haves

  • Fellowship training in Geriatrics, Addiction Medicine, or other professional degrees (e.g., M.B.A., J.D., M.P.H.)
  • 5+ years of experience in an outpatient primary care practice
  • 2+ years of experience practicing in a value-based care model
  • 2+ years of experience in direct management of APCs or physicians.
