The Medical Director at Theoria Medical is responsible for overseeing the care of residents in skilled nursing facilities, collaborating with Nurse Practitioners and Physician Assistants to provide high-quality, patient-centered care. This role offers flexible scheduling with no on-call, weekends, or overnight duties, allowing for a balanced work-life experience. Responsibilities

• Serve as the attending physician for residents in skilled nursing facilities.
• Collaborate with Nurse Practitioners and Physician Assistants for day-to-day resident care.
• Provide comprehensive primary care services by rounding on patients.
• Diagnose and treat patients with acute and chronic illnesses.
• Complete provider documentation in Theoria's proprietary EHR system.
• Participate in Chronic Care Management, Transitional Care, and other medical quality initiatives.
• Supervise collaborating nurse practitioners and physician assistants.
• Utilize electronic medical tools for administrative functions.

Requirements

• Experience in Point-Click Care or similar EHR systems.
• Ability to provide high-quality, patient-centered care.
• Strong collaboration skills with healthcare teams.
• Proficiency in using electronic medical tools.
