About the position

The Medical Foster Home Program Coordinator is a supervisory social worker responsible for enhancing the psychosocial quality of life for veterans, enabling them to remain in their homes or in less restrictive care settings. This role involves program development, recruitment and placement of veteran residents, case management, and coordination with interdisciplinary treatment teams to ensure comprehensive care and support for veterans facing significant challenges.

Responsibilities

• Oversee the Medical Foster Home Program at Salisbury VAMC.
• Develop and implement policies and care processes for the program.
• Recruit and place veteran residents in foster homes.
• Manage casework and provide ongoing psychosocial support to veterans.
• Coordinate with interdisciplinary treatment teams to address complex cases.
• Conduct psychosocial assessments and develop treatment plans for veterans.
• Provide training and mentorship to caregivers and other healthcare providers.
• Market the foster care model to community groups and agencies.

Requirements

• Master's degree in social work from an accredited institution.
• Current state licensure to practice social work at the master's level.
• One year of experience equivalent to the GS-11 grade level in social work.
• Demonstrated advanced practice skills in a specialty area or administration.
• Ability to independently organize work, set priorities, and meet deadlines.
• Knowledge of supervisory duties including clinical supervision and quality improvement.

Nice-to-haves

• Certification or post-master's degree training in social work.
• Experience in a healthcare setting, particularly with veterans.
