About the position

The Medical Reconciliation Pharmacy Technician in the Float Pool at HonorHealth plays a crucial role in enhancing medication safety by assisting pharmacists with the collection and documentation of accurate medication lists for patients admitted to the hospital. This position is essential for optimizing the medication reconciliation process and minimizing medication errors due to incomplete or inaccurate information. The technician will work collaboratively with pharmacists and healthcare providers to ensure a seamless transition of care for patients.

Responsibilities

• Assist pharmacists in collecting and documenting up-to-date medication lists for patients being admitted to the hospital.
• Obtain and verify medication history in the electronic medical record accurately and timely.
• Document medication lists into the electronic health record and communicate with the verifying pharmacist.
• Relay any outstanding transition of care issues to the pharmacist and/or provider promptly.
• Perform other duties as assigned.

Requirements

• High School Diploma or GED required.
• Experience in a pharmacy setting preferred.

Nice-to-haves

• Certification as a Pharmacy Technician (CPhT)
• Experience with electronic health records (EHR) systems.
