About The Position
The Medication Reconciliation Pharmacy Technician prepares detailed home medication lists utilizing a variety of tools. Duties include but are not limited to reviewing medical records for previous medication history, interviewing patients and/or family members, communicating with physician offices and/or pharmacies, and entering medication information into the electronic medical record system. The Medication Reconciliation Pharmacy Technician assists pharmacy with day-to-day compounding and/or dispensing activities and record maintenance when needed. The position works under the supervision of a licensed pharmacist, practicing under the scope of State and Federal laws and statutes. The Medical Reconciliation Pharmacy Technician position is a part of a larger care team based outside the pharmacy, primarily in the Emergency Department and on the acute care floors.
