About the position
The Medical Social Worker in Home Health at CenterWell plays a crucial role in providing interdisciplinary care to home health patients. This part-time position involves evaluating and developing personalized care plans that address the emotional and social needs of patients. The Medical Social Worker also supervises Social Worker Assistants and collaborates with healthcare teams to ensure comprehensive patient care, while adhering to company policies and healthcare regulations.
Responsibilities
- Evaluate and develop personalized care plans for patients based on their emotional and social needs.
- Supervise and provide direction to the Social Worker Assistant as required.
- Assess patients' social and emotional states in relation to their illness or injury.
- Identify and coordinate community resources to assist patients in their recovery.
- Instruct patients and families on coping with social and emotional responses related to health issues.
- Participate in the development and re-evaluation of the physician's Plan of Care.
- Observe, record, and report changes in patients' conditions to the Clinical Manager.
- Participate in discharge planning processes and interdisciplinary care team activities.
- Maintain and submit required documentation, including visit reports and progress notes.
Requirements
- Master's or doctoral degree from an accredited school of social work.
- Social Worker licensure in the state of practice, if required by state law.
- Current driver's license and reliable transportation with insurance.
- Proof of current CPR certification.
- Minimum of one year of experience as a social worker in a healthcare setting, home health, or hospice.
- Knowledge of discharge planning needs and community resources.
- Excellent oral and written communication and interpersonal skills.
