Responsibilities

• Provide direct patient care within a patient's home.
• Complete the initial psycho-social assessment and develop a plan of care for social services.
• Assess the social and emotional factors in order to estimate the patient's capacity and potential to cope with problems of daily living.
• Provide social work intervention through counseling and education for patients to maximize coping capacity.
• Identify and coordinate community resources for the patient.
• Instruct the patient and family to understand, accept and comply with medical recommendations.
• Provide services that promote and restore the patient to optimum social and emotional adjustment within capacity of the patient.
• Assist the physician and other disciplines in understanding the social/emotional factors related to the patient's health problems.

Requirements

• Master of Science degree in Social Work.
• Licensed Bachelor Social Worker / LBSW certification.
• Previous home health, hospice, skilled nursing, rehab facility, hospital, mental health clinic, or child welfare setting experience preferred.
• Familiarity with medical software and equipment - MatrixCare/Wellsky a plus.
• Participation in multi-disciplinary patient case conferences, staff meetings and in-service education programs.
