The Medical Social Worker (Per Diem) position involves providing support and advocacy for clients in a healthcare setting. The role focuses on assessing client needs, collaborating with healthcare professionals, and intervening in crisis situations to ensure clients receive appropriate care and resources. This position is contract-based and requires a valid Clinical Social Worker license.
Responsibilities
- Cooperate with other professionals to evaluate patients' medical or physical condition and to assess client needs.
- Advocate for clients or patients to resolve crises.
- Collaborate with other professionals to assess client needs or plan treatments.
- Investigate abuse or neglect cases and take authorized protective action when necessary.
- Interview clients to gather information about their backgrounds, needs, or progress.
- Intervene in crisis situations to assist clients.
- Refer clients to community or social service programs.
- Refer individuals to educational or work programs.
Requirements
- Clinical Social Worker licensure required.
- Deep understanding of the psychosocial aspects of illness and health.
- Experience with community facilities and resources for discharge planning.
- Strong written and verbal communication skills.
- Strong crisis intervention skills.
- Excellent short-term counseling skills.
