About the position

The Medical Social Worker (MSW) at HarmonyCares plays a crucial role in providing social casework to individuals and families receiving nursing or other services. This position involves assessing social and emotional factors related to health problems, facilitating referrals for financial assistance, and participating in interdisciplinary team conferences to address social issues impacting patient care. The MSW is dedicated to delivering age-friendly care and ensuring that patients' needs are met through coordinated efforts with healthcare teams and community resources.

Responsibilities

  • Provide social casework to individuals and families receiving nursing or other services from the Agency.
  • Assess a family's financial situation and refer to an Agency for financial assistance if indicated.
  • Respond to referrals for casework from Agency staff or external professionals.
  • Participate in Agency IDG team conferences to identify social problems and assess family strengths and weaknesses.
  • Refer patients and families to community agencies with appropriate follow-up.
  • Participate in case conferences with other agencies.
  • Interpret social resources to staff and health services to special agencies.
  • Ensure the use of the 4Ms (What Matters, Medications, Mentation, Mobility) and provide Age-Friendly Care.

Requirements

  • Master's degree (MSW) from an accredited school of Social Work.
  • Current Social Worker License in the state of practice.
  • Valid driver's license and good driving record.
  • Proficiency in clinical skills.
  • Ability to make sound professional clinical judgment.
  • Ability to assess and document patient needs and formulate individualized care plans.
  • Excellent verbal and written communication skills.
  • Proficiency in personal computer use including email, clinical, word processing, and spreadsheet software.

Nice-to-haves

  • Advanced Palliative Hospice Social Worker Certification-APHSW-C.
  • Two years' experience within hospice.
  • Certified in Volunteer Administration (CVA).
  • EMR experience.
