About the position

The position involves providing biopsychosocial assessment, crisis intervention, short-term counseling, advocacy, and linkage to resources for patients and their families/significant others across all ages in various patient care settings. Responsibilities include psycho-education, facilitating support groups, and consulting on psychosocial aspects of care as part of an interdisciplinary team. The role also includes serving as a liaison to community programs and collaborating on discharge planning. Education and training for staff and participation in committees may also be required.

Responsibilities

• Perform biopsychosocial assessments and develop a plan of care for patients and families.
• Provide direct clinical services including discharge planning, individual and family counseling, crisis intervention, and linkage with resources.
• Advocate on behalf of patients and families for services and access to care.
• Document in the patient medical record and other hospital records accurately and timely.
• Establish and maintain positive relationships with other hospital departments and community representatives.
• Foster an environment of mutual support and professional excellence throughout the department.

Requirements

• Master's degree in Social Work.
• 2 years of recent relevant experience.
• Clinical skills in biopsychosocial assessment and clinical interventions.
• Collaboration skills and ability to work effectively on a team.
• Skills in patient and family advocacy.
• Knowledge of transitions of care and community resources.
• Knowledge of behavioral health and skills to assess mental health functioning.

Nice-to-haves

• Knowledge of palliative care and psychosocial needs at end-of-life.
• Knowledge of bioethics and legal issues impacting patient care.
• Proficient in using a computer for data entry and communication.
