About the position
The Medical Social Worker (MSW) plays a crucial role in providing care coordination and support for patients at Cottage Health. This position focuses on assisting psychosocially and medically complex patients during their hospital stays, ensuring successful discharges and optimal care management. The MSW is responsible for conducting biopsychosocial assessments, program development, and resource linkage, contributing to high-quality outcomes and customer satisfaction.
Responsibilities
- Perform care coordination and resource linkage for patients.
- Conduct clinical interventions and program development.
- Carry out utilization reviews and biopsychosocial assessments.
- Facilitate discharge planning for patients admitted to the hospital.
- Assist psychosocially complex patients through their hospital stays.
Requirements
- Master's Degree in Social Work from a CSWE accredited School of Social Work.
- California Children Services (CCS) Paneling within six months of hire.
Nice-to-haves
- 1-2 years of relevant social work experience in a healthcare setting.
