About the position

The Medication Reconciliation Technician at Penn State Health Lancaster Medical Center plays a crucial role in the pharmacy team by ensuring accurate medication documentation for newly admitted patients in the emergency department. This position involves working under the supervision of a pharmacist to prevent medication errors and duplications, facilitating effective communication among healthcare providers, and adhering to relevant laws and regulations.

Responsibilities

  • Obtain and document accurate medication lists for newly admitted patients in the emergency department.
  • Enter medication information into the electronic health record for use by healthcare providers.
  • Assist in determining medication orders during hospital admission and discharge.
  • Provide services within the scope of license, certification, and assessed competencies.
  • Communicate effectively with patients, staff, and outside providers.

Requirements

  • High School Diploma or equivalent required.
  • Current Pharmacy Technician certification or licensed practical nurse by state of employment or holds a multistate LPN license.

Nice-to-haves

  • Minimum three (3) years of medication history or pharmacy experience preferred, including hospital and community experience.
