Responsibilities

• Work directly with children and their families in the community.
• Complete care plans and monitor clients and/or families.
• Assess case management needs for clients and families.
• Complete the TCM assessment to provide effective services.
• Ensure continuity of care through a flexible work schedule.

Requirements

• Bachelor's or Master's Degree in human services, social work, or counseling.
• Experience working with children aged 0-18 and their families.
• Familiarity with TCM (Targeted Case Management).
• Strong organizational skills.
• Experience working with systems and understanding family dynamics.
• Behavior management strategies knowledge.
• Ability to work well with a treatment team.
• Valid driver's license with a clean driving and criminal record.

Nice-to-haves

• Prior experience in a mental health, human service, or school setting.
