About the position

The Clare Bridge Program Coordinator at Brookdale is responsible for planning, coordinating, and facilitating person-centered dementia-friendly programs that enhance the quality of life for residents within the dementia care community. This role involves engaging residents in meaningful activities, ensuring compliance with care standards, and collaborating with community leadership to educate families and staff about dementia care. The position offers opportunities for career advancement and emphasizes personal and professional growth in a supportive environment.

Responsibilities

  • Designs a 7-day a week person-centered program that is engaging and meets the needs and interests of individual residents.
  • Develops a monthly calendar based on residents' shared interests and ensures special events and daily programs are executed as scheduled.
  • Ensures dementia care programs comply with Brookdale Excellence Standards Tool (BEST) and other regulations.
  • Collaborates with community leadership to educate families and associates about Alzheimer's disease and dementia care.
  • May provide direct supervision of Clare Bridge Program Assistant/s, including hiring and performance evaluations.
  • Plans and coordinates special events and programs, including holidays and educational events.
  • Reports changes in residents' health to clinical partners and collaborates on interventions.
  • Oversees social media content to align with Brookdale's culture and standards.
  • Plans and schedules programming events outside the community, which may include driving a community vehicle.
  • Maintains department budget and may manage volunteer programs.

Requirements

  • Bachelor's Degree in therapeutic recreation, gerontology, education, or related field preferred.
  • Minimum two years of direct programming experience with older adults and persons with dementia preferred, or equivalent combination of education and experience.
  • Must have a valid driver's license and a current 3-year driving abstract with no more than one moving violation.
