About the position

The Memory Care Coordinator at Liberty Adult Day Health is responsible for organizing and leading daily memory enrichment activities for individuals with memory impairment. This full-time position focuses on enhancing the quality of life for participants through person-centered care, tailored activities, and support for their cognitive abilities. The role requires empathy, patience, and strong organizational skills to effectively assist participants in their daily routines and activities.

Responsibilities

  • Organizing and leading daily memory enrichment activities for participants.
  • Supporting and improving participants' quality of life with compassion and patience.
  • Encouraging participation and celebrating achievements of memory care patients.
  • Preparing background materials and conducting research for appropriate activities.
  • Assisting with personal care, exercises, toileting, ambulation, and eating.
  • Helping participants safely to and from transportation upon arrival and departure.
  • Assisting the Activities Director and other program staff in operating activity program services.
  • Maintaining daily flow sheets of care provided.
  • Preparing and serving morning and afternoon snacks and assisting with the noon meal.
  • Monitoring the status of participants and informing the RN of any progress, problems, or changes in behavior.

Requirements

  • Dementia certified
  • Prior experience working with memory impaired adults and adults with special needs preferred
  • CPR/First Aid certified
  • Able to pass a criminal background check
  • Able to bend, pull, push, squat, turn, transfer, and lift 50 pounds or more.
