About the position

The Memory Care Leader at The Oaks at Byron Center plays a crucial role in supporting the day-to-day functions of the Legacy Lane, focusing on providing exceptional care and education for residents with dementia. This position requires adherence to current federal, state, and local standards, guidelines, and regulations governing long-term care facilities. The Memory Care Leader will coordinate with the Executive Director and the leadership team to ensure accountability for the daily operations of the Legacy Lane programs within the health campus. This includes managing the Best Friends Forever (BFF) Program, ensuring that Daily Rhythms activity programming is executed according to company standards, and maintaining a dementia-safe environment through regular rounds and management of environmental stimuli. In addition to operational responsibilities, the Memory Care Leader will engage with resident family members, providing them with orientation and guidance through the care continuum. This role also involves interviewing potential team members to ensure they possess the necessary skills to work with residents with dementia. The Memory Care Leader will lead educational efforts for campus staff and the community on dementia-related topics, acting as a Dementia Champion. The position requires strong organizational skills, effective communication, and the ability to maintain confidentiality while providing high levels of customer service. The Memory Care Leader will also oversee meal service in accordance with the company's Family Style Dining Standards, ensuring a positive dining experience for residents. Overall, this position is vital in fostering a supportive and engaging environment for residents with dementia, ensuring that their needs are met with compassion and professionalism.

Responsibilities

  • Coordinates daily operations of the Legacy Lane programs with the Executive Director and leadership team.
  • Manages all aspects of the Best Friends Forever (BFF) Program, including monthly team education.
  • Ensures Daily Rhythms activity programming is occurring per company standards.
  • Maintains Department Leader Rounding binder and partners with Executive Director for follow-up.
  • Assists with maintaining appropriate resident population as per memory care move in/move out criteria.
  • Participates in memory care Resident First Meetings/Service Plans and guides family members through care continuum.
  • Develops and maintains relationships with resident family members for excellent customer service.
  • Provides and reviews Family Orientation Guide with new family members.
  • Maintains open communication with staffing coordinator for consistent dementia staffing and support.
  • Interviews potential Legacy team members to ensure they possess necessary skills for dementia care.
  • Maintains a dementia-safe environment through daily rounds and management of noise and stimuli.
  • Oversees and assists with meal service adhering to Family Style Dining Standards.
  • Leads educational efforts on dementia-related topics and acts as a campus 'Dementia Champion'.
  • Partners with marketing to ensure First Impression Standards are in place and ensures tour readiness.

Requirements

  • Knowledge of operational services terminology and principles of dementia care.
  • Understanding of person-centered care philosophies and Medicare/Medicaid guidelines.
  • Familiarity with healthcare principles, regulations, and compliance with external regulatory agencies.
  • Skills in developing a servant leadership culture and organizing multiple tasks with attention to detail.
  • Ability to communicate effectively, both orally and in writing, and recommend solutions to problems.
  • Experience in assessing resident psychosocial needs and developing care plans based on those needs.
  • Previous work experience in dementia care is preferred.

Nice-to-haves

  • Previous activities experience or certification as C.N.A, QMA/KMA, Med Tech, or LPN/RN license in the state of employment.
