About the position

As a Clare Bridge Program Assistant at Brookdale, you will play a vital role in enhancing the quality of life for residents within the dementia care community. Your primary responsibility will be to assist the Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person-centered, dementia-friendly programs tailored to meet the specific needs of each resident. This position aligns with Brookdale's Resident and Family Engagement philosophy, focusing on providing meaningful and purposeful dementia care programs that engage residents and honor their individual abilities, historical routines, interests, and preferences. In this role, you will support the implementation of programs that cater to the seven domains of well-being: purposeful, intellectual, social, spiritual, emotional, and physical. You will assist in leading small sensory-focused groups for residents who may not be able to participate in larger settings. Additionally, you will review individual resident service plans and complete necessary program documentation in compliance with Brookdale standards and state regulations. Your responsibilities will also include providing guidance to associates on the importance of engagement and programming, coordinating special events, and communicating positively with residents, families, and community professionals about the benefits of Brookdale's dementia care program. You will assist in meeting quality improvement initiatives and may be required to support programming events outside the community, which could involve driving a community vehicle. Furthermore, you will assist with personal care, housekeeping, and dietary responsibilities as needed to maintain service quality according to Clare Bridge Program standards.

Responsibilities

  • Assist Clare Bridge Program Manager or Coordinator in leading and implementing person-centered dementia-friendly programs.
  • Support the implementation of programs that meet individual needs and interests of residents.
  • Engage residents in the seven domains of well-being programs in a dementia-friendly structured environment.
  • Review individual resident's service plans and complete program documentation as required.
  • Ensure dementia care programs comply with state, federal, and Brookdale regulations.
  • Provide guidance and resources to associates regarding programming and engagement.
  • Coordinate and participate in special events at the community, including family socials and open houses.
  • Communicate positively with residents, families, and community professionals about the dementia care program.
  • Assist with quality improvement initiatives including the Brookdale Excellence Standards Tool (BEST).
  • Support plans and scheduled programming events outside the community, including driving a community vehicle if necessary.
  • Assist with volunteer programs, including training and supervising volunteers.
  • Perform personal care, housekeeping, and dietary responsibilities as needed.

Requirements

  • High School Diploma or equivalent required; Associate's Degree preferred.
  • Minimum one year of previous experience with older adults and persons with dementia preferred, or equivalent combination of education and experience.
  • Must have a valid driver's license and a current 3-year driving abstract with no more than one moving violation.

Nice-to-haves
