About the position

The Memory Care Resident Services Coordinator (RSC) plays a crucial role in ensuring the well-being and care of memory care residents at The Springs Living. This position serves as the primary contact for residents and their families, addressing service issues and coordinating care plans. The RSC is responsible for training health services employees, monitoring communication logs, and ensuring compliance with state regulations. This role requires strong leadership skills and a commitment to enhancing the quality of life for residents.

Responsibilities

  • Serve as the primary point of contact for memory care residents and their families regarding resident care.
  • Assist in conducting continuing education and training for health services employees as directed by the Memory Care Administrator or Health Services Administrator.
  • Monitor the Communication Log to ensure health services employees are informed of changes and enter pertinent information as necessary.
  • Advise health services employees concerning resident issues and direct them to notify RN/LPN, Memory Care Administrator, or Health Services Administrator as necessary.
  • Find replacement health services employees for open shifts and cover shifts if unable to find replacements.
  • Create and maintain new resident charts, ensuring they are updated, organized, and purged according to company policy.
  • Enter new residents' personal information into Point Click Care, ensuring accuracy and alert charting assignments are made.
  • Perform chart audits to ensure compliance with state regulations, including POLST form reviews.
  • Counsel employees as needed and meet routinely with residents and their families to review care plans for accuracy and preferences.

Requirements

  • Valid LPN license or equivalent experience in health care services.
  • Experience in supervising health services employees.
  • Proficiency in understanding and implementing resident rights and regulatory requirements.
  • Ability to administer medications accurately and safely.
  • Knowledge of social work principles to support residents' emotional well-being.
  • Leadership skills to guide a team of health services employees effectively.
  • Expertise in creating comprehensive care plans tailored to individual needs.
  • Proficient PC skills including email, record keeping, and routine database activity.

Nice-to-haves

  • Medication Technician experience preferred.
  • Current CPR/First Aid certification.
  • Experience in assisted living and memory care settings.
