About the position
The Bi-Lingual Mental Health Advocate is a full-time role responsible for providing individualized clinical support services to consumers in need of consistent contact to ensure optimal psycho-social functioning. This position is part of the PACT Team, which focuses on case management and specialized services, medication deliveries, and education for the consumers' natural supports, such as family members. The role emphasizes collaboration with consumers and team members to assess needs, develop treatment goals, and monitor service effectiveness.
Responsibilities
- Provide face-to-face individualized clinical support services for consumers.
- Serve as a member of the PACT Team, providing case management and specialized services.
- Participate in PACT medication deliveries.
- Collaborate with consumers and PACT team staff to assess client needs and develop treatment goals.
- Monitor service delivery and evaluate the effectiveness of services and client satisfaction.
- Ensure accurate and timely documentation of services in accordance with policy guidelines.
Requirements
- Bachelor's degree from an accredited college/university in human services or a health-related field.
- Two years of post-Bachelor's experience in the provision of mental health or related services.
- Valid New Jersey driver's license and ability to be insured by the employer.
- Bi-Lingual Spanish speaking.
