About the position

The Mental Health Case Manager at Step Up on Second, Inc. is responsible for providing comprehensive mental health services and case management to adults experiencing chronic homelessness and serious mental health issues. This role involves outreach, housing placement, and coordination of care, ensuring clients receive the necessary support to achieve stable housing and improve their quality of life. The position requires collaboration with other service providers and adherence to recovery-oriented service models.

Responsibilities

• Provide intensive case management services to residents through Permanent Supportive Housing.
• Conduct outreach and engagement, screening, intake, and assessment for housing applications.
• Develop and monitor service plans, ensuring compliance with eligibility and lease-up procedures.
• Facilitate life skills groups and community-based recreational opportunities.
• Work collaboratively with clients to identify needs, set goals, and develop Personal Service Plans.
• Provide ongoing support and crisis intervention, monitoring for warning signs of decompensation.
• Complete all necessary documentation according to MediCAL standards in Electronic Health Record.
• Attend daily staff meetings and participate in supervision and training as required.

Requirements

• Bachelor's degree in psychology or social work, or 3 years of related work experience with homeless populations or those diagnosed with mental illness.
• Experience in crisis intervention and harm reduction.
• Strong computer skills, including proficiency in Microsoft Office.
• Valid California driver's license and insurance with a good driving record.
