About the position

The Mental Health Case Manager at Western Tidewater Community Services Board (WTCSB) is responsible for providing coordinated, recovery-oriented care to individuals with mental health, intellectual disabilities, and substance abuse issues. The role involves assessing consumer needs, linking them to appropriate services, and implementing individualized service plans (ISPs). The Case Manager will also provide education and counseling to consumers and their families, monitor service delivery, and manage consumer records in compliance with licensure and Medicaid standards.

Responsibilities

• Conduct consumer assessments and develop individualized service plans (ISPs).
• Link consumers to appropriate services and resources.
• Provide follow-up instruction, education, and counseling to consumers and families.
• Monitor service delivery through regular contacts with consumers, family members, and service providers.
• Manage consumer records in accordance with licensure and Medicaid standards.

Requirements

• Bachelor's degree from an accredited college or university in Psychology, Social Work, or a related field.
• One year of clinical experience in a Human Services field.
• Valid Driver's License with an acceptable DMV record.

Nice-to-haves

• Experience in Substance Abuse, Mental Health, or Intellectual Disabilities/Developmental Disabilities (ID/DD) services.
