About the position

The Mental Health Community Support Specialist at Kaleidoscope Counseling & Case Management, LLC is responsible for managing a small caseload of families, providing intensive support and utilizing short-term, solution-focused treatment strategies. This role involves conducting comprehensive assessments, preparing service plans, facilitating wraparound services, and providing direct clinical services. The specialist collaborates with various professionals and responds to crisis situations, ensuring that individuals receive the necessary support and resources.

Responsibilities

  • Manage a small caseload of individual families, providing intensive services.
  • Interview and document individuals from multiple perspectives, including family functioning and treatment history.
  • Prepare written service plans incorporating input from individuals and service providers.
  • Facilitate wraparound services to meet service objectives and identify additional resources.
  • Provide direct clinical services, including parenting skills and anger management.
  • Collaborate with other case-involved providers and professionals.
  • Respond to crisis situations with 24-hour availability.
  • Document case activities for medical necessity and billing purposes.
  • Prepare progress and monthly reports as required by referral sources.
  • Ensure individuals' MCOs remain active during service delivery.
  • Participate in interdisciplinary meetings and assessments.
  • Link individuals with external programs and services.
  • Attend court hearings and present testimony as needed.
  • Work with the MHSS Supervisor to enhance therapeutic interventions.
  • Prepare written discharge summaries and plans.
  • Accept and respond to routine and emergency referrals.

Requirements

  • Bachelor's degree in Social Work, Psychology, or Human Services field.
  • At least 1 year of experience working with adults, children, and families with a mental health diagnosis.
  • QMHP-A certification required.
  • Skills in program development and design, diagnostic assessment, and social work principles.
  • Knowledge of community resources and ability to maintain effective working relationships.
  • Effective oral and written communication skills.
  • CPR and First Aid certification required.

Nice-to-haves

  • Experience with motivational interviewing and CBT techniques.
  • Ability to work flexible hours and manage a hybrid remote work environment.
