In this role, you will work closely with primary care providers, and other health professionals to deliver evidence-based mental health interventions. This position requires adaptability, a strong commitment to patient care, and a collaborative spirit.
Responsibilities
- Provide brief, evidence-based therapeutic interventions in a primary care setting.
- Collaborate with primary care providers to address patients' mental health needs promptly.
- Conduct comprehensive assessments, including mental health screenings and risk evaluations.
- Develop and implement individualized treatment plans in coordination with other healthcare providers.
- Engage in crisis intervention and risk management when necessary.
- Offer psychoeducation to patients and their families as needed.
- Document clinical encounters promptly and maintain accurate patient records in compliance with organizational policies.
- Participate in case conferences, staff meetings, and professional development opportunities.
