About the position

The Behavioral Health Specialist plays a crucial role in providing direct patient care for individuals with chronic mental illness and socioeconomic challenges. This position operates within a healthcare team, utilizing the Patient Centered Medical Home Model to coordinate care with multiple primary care providers (PCPs). The specialist is responsible for skilled assessment, education, and collaboration with healthcare and community resources to help patients achieve optimal health and quality of life.

Responsibilities

  • Provide direct patient care for individuals with chronic mental illness and socioeconomic challenges.
  • Work within a healthcare team to coordinate care using the Patient Centered Medical Home Model.
  • Conduct skilled assessments and provide education to patients.
  • Collaborate with healthcare and community resources to assist patients in gaining self-management skills.
  • Support patients in achieving optimum functional health status and quality of life.

Requirements

  • Master's Degree in Social Work or a related field in Human Services or Psychosocial Mental Health.
  • 2 years of clinical behavioral health care experience.
  • Advanced clinical experience in behavioral health care, including evaluation, diagnosis, and intervention of psychiatric and substance use disorders.

Nice-to-haves

  • Ph.D. in Psychology.
  • Experience with Child & Adolescent Mental Health.
  • Experience working in an integrated collaborative environment.
  • Experience with Electronic Medical Records (EMR).
