About the position

The Mental Health Social Practitioner at Fountain House plays a crucial role in supporting individuals with serious mental illness (SMI) by fostering community connections and promoting independent living skills. This position is centered around creating a therapeutic environment within a Clubhouse setting, where staff and members collaborate to enhance recovery and social engagement. The practitioner will engage in various activities, from providing advocacy and support to facilitating vocational training and community integration, all aimed at empowering members to lead fulfilling lives.

Responsibilities

  • Perform member engagement, social design, relationship development, and therapeutic intervention.
  • Support and develop a therapeutic community based on Social Practice and International Standards for Clubhouse Programs.
  • Utilize the community to design positive interventions addressing barriers like social isolation and anxiety.
  • Create access to environments outside of Fountain House to build skills and confidence.
  • Develop relationships with members and the community to support positive change and combat social isolation.
  • Engineer social interactions to create therapeutic results and develop a positive community.
  • Observe and assess member experiences to create specific rehabilitation interventions.
  • Provide daily advocacy and engagement to help members gain self-worth and confidence.
  • Provide counsel for members in individual and group sessions to assist with life adjustments.
  • Provide training support for transitional employment opportunities and maintain relationships with employers.
  • Assist members with entitlements and benefits applications.
  • Facilitate supported housing applications and assist with physical moves into housing units.
  • Advocate for members' well-being and safety regarding clinical, legal, and independent living opportunities.
  • Complete documentation on time and maintain compliance with agency requirements.
  • Maintain confidentiality of member records.

Requirements

  • 1-3+ years' experience working with individuals with serious mental illness or in a community setting preferred.
  • 1-2 years coaching and counseling experience preferred.
  • 1-2 years of vocational rehabilitation or employment specialist experience preferred.
  • Ability to effectively manage member concerns and behaviors.
  • Proven experience with mental health crisis management.

Nice-to-haves

  • Graduate degree or equivalent preferred.
  • Driver's License and the ability to drive a van preferred.
