About the position

The Mental Health Targeted Case Manager position at Douglas Gardens CMHC involves providing comprehensive mental health services to clients in the Miami area. The role focuses on assisting clients in meeting their mental health and social service needs through advocacy, linking, monitoring, and planning for services. This full-time position operates Monday to Friday, with no weekend or after-hours work required, and offers a supportive work environment that fosters learning and personal satisfaction in making a difference in the community.

Responsibilities

• Conduct case management assessments.
• Develop treatment plans and case management service plans.
• Monitor services and progress toward goals, leading to reassessment and development of new service plans or linkages.
• Intercede on behalf of clients to ensure access to needed services and/or resources.
• Link clients to local community resources.
• Complete prior authorizations for services or medications.
• Meet with clients in off-site locations including client's home for bi-monthly home visits.

Requirements

• Minimum requirement of a bachelor's degree in the field of human services - Social Work, Psychology, Criminal Justice or a related discipline from an accredited College or University.
• Minimum one-year of experience in a case management like position, working with a similar client population, preferred.
• Bilingual English/Spanish.
• Successfully complete background screening and drug screening.
• Individual must have a valid Florida Driver's License with clean driving history and automobile liability coverage as required by Florida law.
• Completion of AHCA approved mental health targeted case management training within three months of hire - Provided by the company.
• Attain Florida Certification Board's Certified Behavioral Health Case Manager Certification within six months of hire - application expense reimbursed by employer.
