About the position

The Mental Health Triage Coordinator plays a crucial role in assessing patient needs and managing care within the mental health services at Marathon Health. This position involves conducting risk assessments, providing supportive counseling, and coordinating care plans to ensure patients receive the appropriate level of care. The coordinator will work closely with the clinical team to facilitate patient connections and manage referrals, ultimately aiming to help patients achieve optimal health outcomes.

Responsibilities

  • Triage mental health calls assessing urgency and providing initial support and guidance.
  • Conduct patient assessments and determine the appropriate level of care.
  • Manage and prioritize urgent mental health cases.
  • Collaborate with healthcare providers to develop and implement individualized care plans.
  • Work with other mental health and medical clinicians for integrated patient care.
  • Complete documentation in EMR in a timely manner.
  • Address social needs of patients and utilize appropriate referral sources for specialty care.

Requirements

  • Associate's or Bachelor's in a Mental Health-related field (Social Work, Counseling, Psychology, etc.). Master's Degree preferred.
  • 2 or more years of experience in mental health settings (hospitals, clinics, community organizations) strongly preferred.
  • Experience in crisis intervention, patient assessment, and case management required.
  • Experience in triage, including assessing urgency and determining appropriate care level.

Nice-to-haves

  • Preferred experience in providing mental health services, including evaluation, diagnosis, and treatment of mental illness.
  • Graduate students currently pursuing relevant degrees may also be considered.
