In this full-time position, you will work in a community-based program located in Glendora, with a schedule from Monday to Friday, 7:30 am to 3:00 pm. The role does not require driving, allowing you to focus on providing support and guidance to individuals in a safe and nurturing environment. You will assist individuals with their assessments, planning, and evaluation processes, and help them prepare for meetings with their support teams. Your responsibilities will include developing and implementing Individual Service Plans (ISPs) that align with personal goals, maintaining required documentation, and facilitating person-centered activities that promote life skills learning experiences. As a Mentor/Life Skills Coach, you will supervise individuals during their time at the service, providing personal care support as needed, which may include assistance with nutrition and personal hygiene. You will also support individuals in finding their preferred ways to communicate their needs and choices, educating them on the use of appropriate tools to help them express themselves. This position requires adherence to established procedures, standards, and regulations to ensure the health and safety of individuals and staff, as well as compliance and quality of services. Responsibilities

  • Assist individuals with their assessments, planning, and evaluation process.
  • Support individuals in preparing for and attending meetings with their support team.
  • Advocate and facilitate changes to the Service Plan as necessary.
  • Assist in the development and implementation of an Individual Service Plan (ISP) consistent with the individual's personal goals.
  • Maintain required documentation and data, including ISPs, quarterly and annual evaluations, and case notes.
  • Assist individuals with the selection of person-centered activities and life skills learning experiences.
  • Work with other direct support associates in the planning and delivery of activities consistent with individual and service objectives.
  • Supervise individuals while they are at the service and provide personal care support as required.
  • Support individuals in finding their preferred way to communicate their needs and choices.
  • Educate individuals in the use of appropriate tools to help them find their voice.
  • Safely and effectively use all technology, alternative communication, and adaptive equipment related to the individuals and the service.
  • Train individuals in the use of assistive technology as directed.
  • Follow procedures, standards, guidelines, and best practices established in designated services.
  • Attend in-service trainings and meetings as required.
  • Perform other duties as assigned.

Requirements

  • High school diploma or GED.
  • CPR Certification.
  • First Aid Certification.
  • Experience working with people with disabilities.
  • Basic computer skills.
  • Ability to demonstrate actively listening, patience, and empathy in communication.
  • Ability to be creative and flexible, problem solve, and organize tasks in a timely manner.
  • Ability to interact appropriately and maintain positive working relationships with associates, stakeholders, and individuals.
  • Ability to demonstrate good judgment and decision-making skills.
  • Ability to properly interpret and follow policies, procedures, and regulations.
  • Ability to provide assistance with personal care to participants, including transferring individuals with no weight-bearing skills.
  • Ability to lift up to 50 pounds repetitively and tolerate an outdoor work environment.
  • Ability to travel locally with individuals to participate in activities.

Nice-to-haves
