About The Position

The Mental Health Substance Use (MHSU) Care Manager is a mobile position primarily focused on engaging adults and children/adolescents in mental health and substance use services within the assigned communities of Davidson, Davie, and Forsyth counties in North Carolina. This role involves working closely with community hospitals, providers, and stakeholders to ensure that members receive appropriate assessment, oversight, and services. The MHSU Care Manager is responsible for proactive intervention and care management, which includes treatment planning, assessment, referral/linkage, and monitoring of services. The position requires a strong commitment to providing education and referrals regarding available services and supports, including physical health, behavioral health, intellectual/developmental disabilities (I/DD), long-term services and supports (LTSS), traumatic brain injury (TBI), pharmacy, vision, and dental services. In this role, the Care Manager will link members to necessary behavioral health and physical health care services, facilitating connections to primary healthcare through various community resources. Responsibilities include conducting comprehensive assessments and care plans, leading care team meetings, and continuously monitoring progress towards goals identified in the care plan. The Care Manager will also identify gaps in services and intervene as necessary to ensure members receive appropriate care, oversee care transitions, and maintain accurate tracking of care management activities and outcomes. Collaboration is a key aspect of this position, as the Care Manager will work with community stakeholders to identify system barriers and manage high-risk team meetings in collaboration with various agencies. The MHSU Care Manager must possess a deep understanding of the Diagnostic and Statistical Manual of Mental Disorders and the service array provided through the LME/MCO's network of providers. This role requires exceptional interpersonal and communication skills, proficiency in Microsoft Office, and the ability to manage multiple tasks and priorities effectively. The Care Manager must also demonstrate a high level of diplomacy and discretion in resolving issues and maintaining effective working relationships.
