About the position

The Geriatric Medical Director is responsible for overseeing the medical aspects of participant care within a clinical setting, ensuring quality outcomes and compliance with medical policies. This role involves collaboration with primary care providers and interdisciplinary teams to deliver comprehensive care to frail older adults, while also participating in community education and support.

Responsibilities

  • Provide general medical direction and supervision of participant care.
  • Oversee clinical outcomes and the Quality Assurance Performance Improvement (QAPI) program.
  • Develop and maintain medical policies, procedures, and standards.
  • Supervise related Primary Care Providers (PCPs) and Advanced Practice Providers (APPs).
  • Conduct history and physical examinations on new participants and periodic reassessments.
  • Develop medical treatment plans and provide necessary services in various locations.
  • Participate in the Intake and Assessment decision for new applicants and their caregivers.
  • Discuss medical conditions and prognosis with participants and caregivers to assist in future care planning.
  • Collaborate with the nursing treatment program for participants.
  • Participate as an active member of the PACE Organization interdisciplinary team.
  • Educate participants and families about health maintenance and chronic conditions.

Requirements

  • Minimum of 1 year experience working with the frail or elderly population.
  • Proficient in Microsoft Suite (Word, Excel, PowerPoint) and basic software for job functions.
  • Excellent organizational, customer service, flexibility, and problem-solving skills.
  • Valid driver's license, proof of insurance, and means of transportation.

  • Effective skills in physical assessment and chronic disease management for frail older adults.
