About the position

The Mobile Crisis Adult Case Manager will provide essential support services and interventions for individuals experiencing mental health or substance use-related crises. This role involves face-to-face interactions within the home and community, utilizing evidence-based practices to assess, stabilize, and refer clients to appropriate services. The position requires collaboration with various departments to ensure effective transitions of care and operates on a 24/7 schedule, including nights and weekends.

Responsibilities

• Provide face-to-face crisis intervention services both on-site and off-site.
• Assess clients' strengths and mental health needs accurately and objectively.
• Establish trust between clients and treatment team members.
• Assist in the implementation of goals and objectives identified in treatment plans.
• Engage individuals in the crisis planning process and complete safety plans.
• Follow up with individuals within two days of the initial crisis intervention.
• Provide referrals to community services and medication management as needed.
• Participate in team staffing and clinical supervision meetings.
• Complete required assessment screenings, including the Columbia Suicide Rating Scale (CSSRS).
• Collaborate with the treatment team to increase patient engagement.

Requirements

• Bachelor's degree in social work, psychology, or related human services.
• Previous related experience preferred.
• Valid driver's license and personal auto insurance.

• Experience with therapeutic interventions for individuals diagnosed with serious mental illness.
• Knowledge of community resources and services for mental health and substance use disorders.
