About the position

The Mobile Nurse Practitioner role involves providing healthcare services primarily to elderly patients in nursing homes and clinical sites. The practitioner will supervise and manage nurse practitioners and clinical staff, perform wellness assessments, provide urgent care, and ensure effective transitions of care for patients returning from hospitals or emergency departments.

Responsibilities

• Supervise and manage nurse practitioners assigned to the location.
• Perform annual wellness assessments on all assigned site participants.
• Provide urgent care services to participants and facilitate same-day access.
• Assess and manage participants with acute changes in condition in a timely manner.
• Assess participants upon return from the hospital/Emergency Department within 5 days, including medication reconciliation and treatment plan coordination.

Requirements

• Licensed to practice as a Nurse Practitioner in NY.
• DEA Certification.
• AANP or ANCC Board Certification.
• 1+ years of experience working with the geriatric population.
• 2+ years of experience in clinical nursing practice in-home, care management, or related field.
