About the position

The Mobile Primary Care Nurse Practitioner role involves providing high-quality primary care services to geriatric patients in assisted living and independent homes across Brevard and Indian River counties. This position is part of a growing practice that emphasizes in-home visits, allowing practitioners to deliver care in a comfortable environment for patients. The role is full-time and contract-based, with a hybrid work model.

Responsibilities

• Provide primary care services to geriatric patients in their homes.
• Conduct in-home visits primarily in the Barefoot Bay area.
• Collaborate with a team of practitioners and support staff to ensure comprehensive patient care.
• Manage on-call responsibilities from 5 PM to 9 AM once every 10 weeks.

Requirements

• Active Nurse Practitioner license in the state of Florida.
• Experience in geriatrics or primary care.
• Ability to work independently and manage patient care in home settings.
