About the position

The Mobile Team Wellness Specialist at Concern Housing is responsible for providing essential support and resources to individuals and families, focusing on enhancing their daily living skills and overall well-being. This role involves linking clients with community resources, offering counseling, and ensuring compliance with program requirements, all aimed at fostering personal growth and independence.

Responsibilities

• Facilitate residents' linkage with community services such as case management and vocational services.
• Develop comprehensive needs assessments and support plans for clients.
• Meet with clients to develop skills and review their progress.
• Maintain all documentation required for client records, including progress notes and discharge plans.
• Provide individual counseling and support services to clients.
• Manage medication and provide training related to medication monitoring.
• Engage in recovery planning using Wellness, Recovery, and Action Planning techniques.
• Conduct daily living skills training, including money management and cleaning.
• Attend supervision and staff meetings regularly.
• Complete other duties as assigned by the Team Leader.

Requirements

• Bachelor's Degree preferred in Psychology, Rehabilitation, Social Work, or a related mental health discipline.
• 2 years of experience working in a similar setting or with individuals with mental illnesses.
• Strong verbal and written communication skills.
• Ability to make sound judgments regarding client care.

Nice-to-haves

• Experience in case management or clinical services.
• Familiarity with community integration and entitlements.
