About the position

The MOMS Home Visitor at Family & Children's Agency (FCA) is responsible for conducting home visits for families with children aged birth to eight years. This role focuses on child development, enhancing parenting skills, and fostering positive parent-child relationships. The Home Visitor also provides case management, connects families with community resources, and facilitates group programming.

Responsibilities

  • Provide home visiting and case management services to a caseload of 10 to 12 families living in the Greater Norwalk Area.
  • Facilitate groups and workshops as assigned; assist with transportation and/or childcare to groups/events as needed.
  • Outreach to identify and recruit new clients for the MOMS Program.
  • Connect clients with appropriate referrals to community resources, including health and social services.
  • Accompany clients to medical and agency appointments as necessary.
  • Serve as an advocate for clients with schools, health care, and agency providers.
  • Provide accurate and relevant information on child development and child health and wellness.
  • Identify client goals and work towards the attainment of client goals.
  • Assist client in strengthening parenting skills and problem-solving skills.
  • Help clients identify and use their informal support network.
  • Participate in orientation and training sessions.
  • Attend regularly scheduled supervision and staff meetings.
  • Attend community and state-wide meetings and trainings as assigned.
  • Complete all assigned paperwork in an accurate and timely manner.
  • Maintain client records in online database.

Requirements

  • Bilingual in Spanish and English.
  • Valid driver's license and reliable transportation.
  • Bachelor's degree preferred; Associate's degree in human services, social work, early childhood education or related discipline required.
  • Knowledge and expertise in child development, parenting, community resources, outreach, and prenatal/postpartum health care.
  • Awareness of cultural and socioeconomic characteristics of clients served.
  • Experience facilitating groups preferred.
