About the position

The Social Work Care Manager at AdventHealth Porter plays a crucial role in supporting patients with complex psychosocial needs. This position involves assessing patient needs, coordinating care transitions, and facilitating discharge planning to ensure efficient and effective patient care. The Social Work Care Manager collaborates with an interdisciplinary team to provide patient-centered care and education on resource utilization and care coordination.

Responsibilities

• Intervene with patients who have complex psychosocial needs.
• Assist with eligibility determination for social programs and funding sources.
• Provide crisis intervention to patients and families with psychosocial needs.
• Coordinate and facilitate the development of discharge plans for high-risk patient populations.
• Receive referrals from interdisciplinary team members for at-risk populations.
• Ensure patient-centered care coordination through the continuum of care.
• Monitor resources and escalate clinical care as needed.
• Conduct patient evaluations of post-hospital needs and develop transition of care plans.
• Communicate daily with the interdisciplinary team during multidisciplinary rounds.
• Educate nurses, physicians, and the interdisciplinary team on resource utilization and care coordination.

Requirements

• Master's degree in Social Work (MSW).
• Minimum of three (3) years experience in hospital/medical social work.
