About the position

The Social Worker position at MacNeal Health Network is designed to support an integrated and comprehensive Performance Improvement program. The role involves assessing, monitoring, evaluating, and improving processes that impact the quality of patient care while ensuring compliance with organizational standards and regulations. Social workers in this role will address the psychosocial needs of patients and their families, particularly in relation to emotional, social, and economic stresses caused by illness, and will assist with complex discharge planning.

Responsibilities

• Assess and provide intervention concerning disability resulting from emotional, social, and economic stresses of illness.
• Identify and provide interventions to meet the psychosocial needs of patients and their families.
• Support complex discharge planning needs.
• Monitor and evaluate processes affecting the quality of patient care.
• Ensure compliance with MacNeal Health Network mission, values, and regulatory standards.

Requirements

• A Master's Degree in Social Work.
• Minimum of six (6) months of prior social work experience in a hospital setting.
• Current certification by the State of Illinois as a Licensed Social Worker (L.S.W.).
