About the position

The MSW Social Worker position involves working collaboratively with a multidisciplinary team to provide high-quality, patient-centered care as part of the Care Coordination program. The role focuses on addressing social determinants of health and supporting safe transitions of care for patients and families, particularly those who are high-cost or high-risk.

Responsibilities

• Support high-cost/high-risk patients to address barriers to care and navigation challenges across the care continuum.
• Provide psychosocial assessment and structured support to help address social and economic barriers to positive health outcomes.
• Apply best practice interventions based upon care standards and ensure behavioral and psychosocial needs are addressed.

Requirements

• Master's Degree in Social Work
• Current LSW license in the State of New Jersey in good standing
• 3-5 years of experience in a health-related field, preferably in a hospital setting
• Strong knowledge of community resources and social service systems
• Familiarity with EPIC EMR preferred
• Behavioral Health background preferred
