Responsibilities

- Determine patients' ability to cope with daily living problems and provide necessary support.
- Help patients and their families understand and implement medical recommendations.
- Assist patients in utilizing community resources to achieve optimal functioning.
- Identify gaps in community resources and promote their development or improvement.
Requirements
- Master's Degree from a school of Social Work accredited by the Council of Social Work Education.
- One year of social work experience in a healthcare setting.
- Current CPR certification.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle.
