About the position

The MSW-Social Worker at WellBe Senior Medical is responsible for providing compassionate care to elderly patients with chronic and acute illnesses in their homes. This role involves delivering primary, urgent, and acute care, as well as counseling, while connecting patients and their families to necessary support services. The MSW will collaborate with a multidisciplinary team to ensure quality care and maintain accurate patient records.

Responsibilities

  • Deliver primary, urgent, and acute care to elderly patients in their homes.
  • Connect patients and their families to support services and community resources.
  • Maintain accurate and current patient records.
  • Schedule and administer initial, urgent, and follow-up appointments as required.
  • Conduct telephonic outreach to patients and providers for assistance with community resources.
  • Develop care plans based on patient requests and assessment outcomes.
  • Collaborate with the interdisciplinary team to address patient psychosocial needs.
  • Document assessments and patient encounters following WellBe standards.
  • Build rapport with patients and their families, relaying concerns to the provider.
  • Coordinate with healthcare providers and community resources to create comprehensive treatment plans.
  • Ensure compliance with regulatory agencies and clinical guidelines.
  • Participate in program and policy development for the Social Work program.

Requirements

  • Master's Degree in Social Work (MSW) required.
  • Two years of social work experience, preferably in healthcare.
  • Minimum one year of experience working with a frail/elderly population.
  • Valid driver's license and access to a car with active auto insurance policy.
  • Strong computer skills, including proficiency in Word, Excel, and PowerPoint.
  • Ability to make psychosocial assessments and develop care plans.
  • Knowledge of community services for the elderly and their families.
  • Strong verbal, written, and interpersonal communication skills.
  • Ability to work effectively in a team environment.

Nice-to-haves

  • Experience writing assessments and reports to monitor client progress.
  • Familiarity with EHR medical documentation.
  • Ability to provide after-hours services as needed.
