This position involves assessing, diagnosing, and treating patients' mental and social conditions, providing counseling, and coordinating care with healthcare teams and community resources. Responsibilities

• Assist patients and families in coping with problems resulting from severe or long-term illness.
• Assess, diagnose, and treat patients' mental and social conditions.
• Counsel individuals and/or families regarding their situations.
• Update case records and document patient interactions.
• Instruct healthcare team members on community resources available to assist patients.
• Plan for continuity of care with hospitals and community agencies.
• Assess and treat social and emotional factors related to patients' illnesses.
• Help patients and significant others understand and implement medical recommendations.

Requirements

• Master's Degree from a school of Social Work accredited by the Council of Social Work Education.
• One year of social work experience in a healthcare setting.
• Current CPR certification.
• Current Driver's License and vehicle insurance.
• Access to a dependable vehicle or public transportation.
• Licensed as a Social Worker from the State Board of Social Work in Texas.
