About the position

The MSW-Social Worker at WellBe Senior Medical is responsible for providing compassionate care to elderly patients with chronic and acute illnesses in their homes. This role involves delivering primary, urgent, and acute care, as well as counseling to patients facing social and behavioral challenges. The MSW will collaborate with a multidisciplinary team to develop care plans, connect patients and families to support services, and maintain accurate patient records, all while ensuring compliance with clinical guidelines and regulatory standards.

Responsibilities

  • Deliver primary, urgent, and acute care to elderly patients in their homes.
  • Conduct telephonic outreach to patients and providers for community resources and social determinant of health needs.
  • Develop and implement care plans based on patient assessments and requests.
  • Coordinate community resources for patients facing psychosocial problems.
  • Collaborate with the interdisciplinary team to address patient psychosocial needs.
  • Maintain accurate and current patient records and documentation.
  • Build rapport with patients and their families, relaying concerns to the care team.
  • Ensure compliance with regulatory agencies and WellBe clinical guidelines.
  • Participate in program and policy development for the WellBe Social Work program.
  • Engage in community involvement activities that link WellBe to its communities.

Requirements

  • Master's Degree in Social Work (MSW) required.
  • Two years of social work experience, preferably in healthcare.
  • Minimum one year of experience working with a frail/elderly population.
  • Valid driver's license and access to a car for patient visits.
  • Strong computer skills, including proficiency in Microsoft Word, Excel, and PowerPoint.
  • Ability to make psychosocial assessments and develop care plans.
  • Strong verbal, written, and interpersonal communication skills.
  • Knowledge of social work principles, case management, and counseling techniques.

Nice-to-haves

  • Experience with electronic health records (EHR) documentation.
  • Familiarity with community services for the elderly and their families.
  • Ability to provide after-hours services as needed.
