About the position

We are seeking compassionate and diligent Licensed Practical Nurses (LPN) to join our dedicated team at a 5 Star Healthcare Facility in Gravesend, Brooklyn, NY. This role involves providing outstanding care to patients, executing physician's treatment plans under the guidance of an RN Supervisor, and serving as a health educator to families and the community. Responsibilities

• Provide outstanding care to patients in accordance with the physician's plan of treatment.
• Execute treatment plans under the guidance of the RN Supervisor.
• Advocate for the well-being of patients and educate families about health matters.
• Ensure compliance with all State and Federal mandates for employment in a Nursing Facility.
• Conduct patient assessments and demonstrate clinical proficiency.

Requirements

• Valid LPN license to practice in New York.
• Current CPR (BLS or ALS) certification.
• Meets all State and Federal mandates for employment in a Nursing Facility.
• Proficient in patient assessment with demonstrated clinical proficiency.
• Strong communication skills to interact effectively with various stakeholders.
