About the position

The position involves overseeing the Social Care Network (SCN) project as part of New York's 1115 waiver initiative, focusing on health equity reform. The role requires managing relationships with regional entities, ensuring compliance with health care requirements, and operationalizing various health programs. The individual will engage with cross-organizational teams to identify strategic needs and track project deliverables, ultimately contributing to the improvement of health-related social needs services.

Responsibilities

  • Serve as Subject Matter Expert (SME) on the 1115 waiver Social Care Network project, Making Care Primary, and Global Budgeting Initiative.
  • Oversee relationships with NY Regional Social Care Network Lead Entities to support New York Health Equity Reform Waiver priorities.
  • Operationalize SCN, MCP, and GB requirements to ensure timely and compliant rollout of UHC initiatives.
  • Engage cross-organization business teams to identify key issues, gaps, strategic needs, and priorities.
  • Develop, maintain, and track project plans, roadmaps, schedules, and other core project management deliverables for the HRSN program.
  • Summarize and communicate proposed processes and changes to address priorities, demonstrating needs through data and assessing impacts.
  • Provide program updates to an executive-level audience.
  • Review and measure deliverables to ensure quality standards are met.
  • Recommend actions to direct analysis and solution of problems.
  • Develop and maintain relevant policies, procedures, job aids, and training materials.

Requirements

  • 5+ years of combined experience in Program Management, Healthcare or Managed Care Analytics, and/or Provider Network.
  • 3+ years of proven project or program management experience managing major projects integrating multiple business areas or systems.
  • 1+ years of proven leadership and matrix partner collaboration skills.
  • Advanced experience in defining and tracking program deliverables including project plans and milestones.
  • Advanced user experience with MS Office Suite (Word, Excel, PowerPoint, Teams, and Visio).
  • Demonstrated ability to meet commitments, build consensus, negotiate resolutions, and garner respect from teams.
  • Exhibit diplomacy with solid negotiation and conflict resolution skills.
  • Ability to travel within New York state up to 10% of the time.

Nice-to-haves

  • PMP, Six Sigma, or other certification programs.
  • Networking Management or Contracting experience.
  • New York State Medicaid experience.
  • Medicaid Managed Care experience.
  • Social Determinant of Health Program Management experience.
  • Proven history of successful process improvement.
