About The Position

Silver Heights Skilled Nursing and Rehabilitation Center is seeking a dedicated Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide high-quality patient care in a family-oriented environment. This role involves overseeing the day-to-day health and wellness of patients, ensuring compliance with regulatory guidelines, and participating in the implementation of company initiatives. The LPN/LVN will work closely with patients, their families, and the healthcare team to deliver effective nursing care and education.

Requirements

• LPN or LVN License is required.
• Previous experience in a patient care setting is preferred.

Responsibilities

• Utilize the nursing process for assessment, planning, implementation, and evaluation of patient care.
• Assess and document the patient's current health status and medical history.
• Plan and implement nursing actions based on patient needs and formulate a plan of care.
• Evaluate and revise the plan of care based on new data and patient condition.
• Document patient care reflecting nursing interventions and patient responses.
• Communicate with physicians regarding patient needs and nursing assessments.
• Collaborate with other health team members to ensure patient wellbeing.
• Teach patients and their families appropriate health information and provide educational materials.
• Administer medications according to policy and document patient responses.
• Demonstrate technical skills when administering treatments and procedures.
